Please find what we are calling our "Weekly Safety Check" questionnaire below. It is super short and quick.

**We need responses for any swimmer attending Fish Camp this week prior to noon tomorrow 4/12**

A response is required by noon Monday EACH WEEK for your swimmer. If we do not have a response from you, you will be asked to fill out a paper copy or your swimmer will not be able to swim.  

Also as a reminder-if in the unfortunate case your child tests positive regardless of symptoms, please do not send your child to practice and let a board member know ASAP.
